Purchase Terms

The SpadeRx Purchase Terms ("Purchase Terms") govern the terms of sale for any products or services purchased from SpadeRx. Certain products or services may also be subject to additional terms and policies. By purchasing from us, you agree to these Purchase Terms, any applicable additional sales terms, and SpadeRx’s Terms and Conditions, which include an arbitration provision and a waiver of class-action claims. These Purchase Terms are considered Additional Terms under the Terms and Conditions.

Offers of Products and Services

SpadeRx reserves the right to refuse service to anyone, at any time, for any reason. We may limit, prohibit, or cancel orders for products or services at our sole discretion. If an order is canceled or limited, we will attempt to notify you via the contact information provided at the time of purchase.

SpadeRx may refuse or cancel orders, regardless of whether the order has been confirmed or your payment method has been charged. We reserve the right to limit quantities purchased per person, household, or order and may implement these restrictions on accounts using the same payment method or billing/shipping address.

Receiving an order confirmation does not signify SpadeRx’s acceptance of your order. We reserve the right to accept or decline orders after they are received. Additionally, we may fulfill orders with reduced quantities or require additional verification before confirming your purchase.

Prices and Payment

Prices for products and services are subject to change without notice. We reserve the right to discontinue or modify products or services at any time. If a product or service is listed with an incorrect price or description due to an error, we reserve the right to cancel the order.

By completing a purchase, you confirm that you are authorized to use the selected payment method and authorize us to charge the total amount, including taxes. If your payment cannot be verified, your order may be canceled.

Our treatments are elective, non-insured procedures. Payment is due prior to services being rendered, and SpadeRx does not submit claims to insurance providers.

Customers are solely responsible for any fees and taxes associated with their purchases.

Chargebacks

A chargeback is a reversal of a payment made using your payment method. If you dispute a charge without contacting us first, SpadeRx reserves the right to cancel pending orders and pursue remedies under applicable law. If you initiate a chargeback and it is deemed unnecessary, you may be held responsible for the amount of the chargeback, along with any associated fees. Please contact us immediately if you believe a charge was made in error.

Services

SpadeRx offers a variety of aesthetic services. Details about each service can be found on our website under FAQs. We require customers to follow all pre- and post-treatment instructions to ensure optimal results. Non-compliance may result in additional treatments or charges for which the customer is responsible.

Cancellation Policy

We require at least 24 hours’ notice to cancel or reschedule an appointment. Failure to provide adequate notice or failure to show up for a scheduled appointment will result in forfeiture of any prepaid services associated with the booking.

Returns and Refunds

All sales are final. Treatments included as complimentary with other purchases are non-refundable and have no monetary value. Should SpadeRx, at its sole discretion, decide to issue a refund, such refunds may only be issued as store credit. Unused or unrendered treatments cannot be transferred to another customer.

Submission of a medical questionnaire, payment, participation in a telehealth consultation, or completion of a Good Faith Exam (GFE) does not guarantee that a prescription will be issued. Not all individuals qualify for treatment, and treatment decisions are made solely by the evaluating provider.

Compounded medications are prepared by licensed compounding pharmacies pursuant to a valid prescription issued by a licensed healthcare provider. Compounded medications have not been evaluated or approved by the U.S. Food and Drug Administration (FDA) for safety, effectiveness, or quality and may differ from FDA-approved medications in quality, safety, efficacy, risks, and side effects.

Novo Nordisk Inc. and Eli Lilly and Company are the only U.S. manufacturers of FDA-approved semaglutide (Rybelsus®, Ozempic®, Wegovy®) and tirzepatide (Zepbound®, Mounjaro®), respectively. These manufacturers do not supply their medications for compounding purposes.

Prescriptions, when issued, are filled and dispensed by independent licensed pharmacies. Spade RX, PC does not manufacture medications, compound medications, operate a pharmacy, or control pharmacy operations.

Weight loss, wellness, and treatment outcomes vary among individuals. No specific result, amount of weight loss, improvement, or treatment outcome is guaranteed. Individual results depend on many factors, including medical history, adherence to treatment, lifestyle factors, and provider recommendations.

Spade RX, PC does not prescribe, dispense, distribute, or facilitate the prescribing of controlled substances. All treatment recommendations and prescription decisions are made solely by licensed healthcare providers based on medical necessity, patient eligibility, and applicable state and federal laws.

Services, providers, medications, and treatment options may not be available in all states. Availability is subject to provider licensure, pharmacy restrictions, clinical appropriateness, and applicable state laws and regulations.

For convenience and branding purposes, this website may refer to “SpadeRX” or “Spade Rx.” Unless otherwise indicated, all references to “SpadeRX,” “Spade Rx,” “we,” “us,” or “our” refer to Spade RX, PC, the professional medical corporation responsible for providing medical services through this platform.

SpadeRX PC currently provides these services only to patients located in the State of California. Services are not currently available in any other state.
